A change request works best when the request, the price and the decision sit on one form. That way the reasons for a change and the agreed cost and time are kept together for the final account.
Who it is for
It is written for quantity surveyors, commercial managers and contractors running JCT projects who want a consistent, auditable paper trail from first valuation to final account, without building their own spreadsheets and letters from scratch on every job.
How to use it
Part one records the change requested. Part two is completed by the contractor with its price and time effect. Part three records the decision: accepted, rejected or revised. The completed form can be logged in the Variation Register.
